WHAT INFORMATION DO WE COLLECT?
We collect information from you when you register on our site, place an order, subscribe to our newsletter, respond to a survey or fill out a form.
When ordering or registering on our site, as appropriate, you may be asked to enter your: name, e-mail address, credit card or debit card information (Please note that we do not store any credit/debit card information on our servers). (b) information relating to any enquiries you make concerning our template documents or purchases you make of our template documents (which may include your name, address, email address, telephone number, company name, role/position, payment details and details of the proposed use or uses of the template documents);
(c) information relating to or supplied to us in connection with any legal services we may provide to you;
(d) information that you provide to us for the purpose of subscribing to our email notifications and/or newsletters (your name, email address and areas of interest); and
(e) any other information that you choose to send to us.
USING YOUR PERSONAL DATA

We do not sell, trade, or otherwise transfer to outside parties your personally identifiable information.
In order to provide the service we do between our customers and their clients it is important for our programming and development team to be able to access all areas of the MyPhysioRehab application. This includes access to client information such as names and email addresses. We also need to access rehabilitation plans that have been created by users if we are required to troubleshoot programming errors or usability issues reported by those users. Any access to such data is made in strict confidence and we maintain both external developer and internal confidentiality agreements (NDA), signed by both all internal staff and external programming support to ensure no data is removed from our system nor passed on in any form to third parties. In addition, we may disclose information about you:
(a) to the extent that we are required to do so by law;
(b) in connection with any legal proceedings or prospective legal proceedings;

You may instruct us not to process your personal data for marketing purposes (by email or otherwise) at any time. SECURITY OF YOUR PERSONAL DATA
We implement a variety of security measures to maintain the safety of your personal information as well as your client /patient information when you place an order or access your personal information. Clients/Patients are sent a secure email containing an encrypted link to the page that hosts their personalised rehab plan. After a transaction, your private information (credit card details, billing address etc.) will not be stored on our servers.We will take reasonable precautions to prevent the loss, misuse or alteration of your personal information.
